I give Redemption Church the right to investigate all references and to secure additional information about me, if job-related.  I hereby release from liability Redemption Church and its representatives for seeking such information and all other persons, corporations, or organizations for furnishing such information.

The information contained in this application is correct to the best of my knowledge.  Should my application be accepted, I agree to be bound by the constitution, by-laws, and policies of Redemption Church (including the Volunteer Covenant).  I understand that failure to keep any of the requirements of my volunteer position is grounds from dismissal.

If over 18:

I also hereby authorize Redemption Church and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and/or investigative consumer report to be generated for volunteer and/or employment purposes.  (All necessary information for the background check are submitted by applicants through Chekr)

*I understand that Redemption Church will obtain a background check through Chekr.  An email with a link to complete Chekr forms will be sent to the email address I provided.  I understand that it is my responsibility to submit all necessary information to Chekr to complete the background check, which includes necessary authorization and releases.

*I agree that while I am serving I will immediately notify Redemption Church if anything occurs which would change the results of my background check and possibly disqualify me to continue in a volunteer position.
